Ways to Enroll 24-25

Enroll At Woodland Hills Academy for the 2024-25 School Year

 

 

All Families Enrolling at WHA Must Complete A WHA Enrollment Packet

 

– STEP 1 - Complete Student Enrollment Packet forms online

 

– STEP 2 - Gather scans or photos of the Verification Documents

 

– STEP 3 - Submit hard copies of all forms to the Main Office beginning April 25th

 

You may complete the forms below online, print and bring them to the school to enroll. Or, come to the school to pick up an enrollment packet.

* For resident enrollment, you must show a current DWP or gas bill to verify your home address to receive an enrollment packet. 

 

 

NEW TO LAUSD ENROLLMENT REQUIREMENTS

If your student has never previously attended an L.A. Unified school, you will need to complete the following forms and bring them IN PERSON to Woodland Hills Academy’s Attendance Office.

  • Enrollment Form (Eng/Spa)
  • Emergency Card (Eng/Spa)
  • Student Housing Questionnaire (Eng/Spa)
  • Publicity Authorization & Release Form (Eng/Spa)

 

As part of the enrollment process, please also review and sign our school rules and policies here.

 

You will also need to provide the following additional documentation at the time of enrollment:

Proof of Residence

Please submit ONE of the following acceptable documents:

  • Current Utility bills or service contract (Gas, Water, or Electricity)
  • Property taxes, rental or lease agreement, current rental receipt with address of property on receipt
  • Official government mail (CalWORKS, Social Security, Medi-Cal)

Proof of Age of Minor

Please submit ONE of the following acceptable documents:

  • Birth certificate/Baptismal certificate
  • Passport
  • Court order
  • Health office/vital statistics record of birth certificate date
  • Department of Public Social Services (DPSS) letter verifying birthdate and an explanation of how this was verified

Parent/Guardian/
Educational Rights Holder/Caregiver
Identification

  • Adult’s government-issued photo ID (Driver’s license or DMV ID)

If Other than Parent is Enrolling

  • Court order establishing the parent/guardian relationship

School Records

  • Transcripts, most recent grade reports, or clearance form with recent grades
  • If your child is currently receiving, or has ever received special education or Section 504 services, please notify us and provide a copy of most recent Individual Educational Plan (IEP) or Section 504 Plan

Proof of Immunization

For your reference, the Parents’ Guide to Immunizations Required for School Entry is available HERE.

Additional Documents (if applicable)

If applicable, please submit copies of:

  • Department of Children and Family Services (DCFS) 1399 Form or minute order
  • Department of Probation placement information and/or Juvenile Court minute order
  • Court orders regarding legal custody, physically custody, or restricted contact with the school or child

 

TRANSFERRING FROM ANOTHER LAUSD ENROLLMENT REQUIREMENTS

If your student is coming from another L.A. Unified school, you will need to complete the following forms and bring them IN PERSON to Woodland Hills Academy’s Attendance Office.

  • Enrollment Form (Eng/Spa)
  • Emergency Card (Eng/Spa)
  • Student Housing Questionnaire (Eng/Spa)
  • Publicity Authorization & Release Form (Eng/Spa)
